Patient Resources

We encourage all patients to fill out their patient forms prior to their visit. At home, you have access to the resources you need to make sure you fill out your forms accurately. Bring your completed patient forms to your first visit to save time and stress.  Also be sure to bring a current valid ID and proof of insurance to your first visit.

What to Expect

On your first visit, you can expect to be welcomed like a member of our extended family. Our dental care team will file your paperwork and familiarize you with our office. Your first visit will include a thorough oral health exam. Dr. Pruitt will evaluate your overall oral health and the condition of your jaw joints, teeth, and gums. Your first visit may include:

  • Bite & Occlusal analysis
  • Review oral health history
  • X-rays
  • Screen for oral cancer
  • Screen for tooth decay or damage
  • One-on-One dentist consultation
  • Teeth Cleaning (if time allows)

Every patient receives a one-on-one consultation with Dr. Pruitt. He will spend time with each patient and will discuss any oral health concerns, cosmetic goals and potential treatment options.
